Transitional Housing- STREET TO HOME PROJECT (S2H)

WHAT WE DO

Atlanta Outreach Project, Inc (AOPI) is committed to being a part of the Metro Atlanta United Way’s Regional Commission on Homelessness (RCOH) street outreach. Using the S2H Housing First Model we house chronically homeless adult men and women directly from the streets with little to no screening. AOP believes that this model is very effective because it eliminates the lengthy referral process that many agencies use, and therefore decreases the amount of time an individual remains homeless and on the streets. The only requirements are that the client is chronically homeless and is motivated to reach self sufficiency.

HOW WE DO IT

AOPI provides apartments and houses that are fully furnished which are shared by 4-6 clients per location. Clients are provided bedding, personal grooming supplies, cleaning supplies and comprehensive case management. AOPI staff meets one-on-one with clients one or more times per week to ensure that they are working towards self sufficiency. We provide referrals for assistance with mental health treatment, substance abuse treatment, job assistance, continuing education and help with applying for disability benefits if needed.

WHY WE DO IT

The ultimate goal of AOP is to secure permanent supportive housing or independent living and provide follow-up services for a minimum of one year.

Permanent Housing- LIGHTHOUSE PROJECT

HOMEWARD BOUND

Atlanta Outreach Project, Inc. (AOPI) provides permanent supportive housing to the chronically homeless men and women in Atlanta who have established a form of income. The majority of our LIGHTHOUSE PROJECT clients come from our S2H transitional program. Once a client has the ability to afford housing, AOPI provides shared apartments and houses to our clients for a flat monthly program fee which includes housing and utilities. AOPI staff meets with clients at least once a month to ensure that the clients are continuing to work towards self sufficiency. This is a cost effective model because it is self supported by the clients. Just a little money up front enables AOPI to offer an immediate and affordable housing option to those who receive a fixed income such as SSI or SSDI or have obtained full or part time employment.

THE ULTIMATE GOAL

The ultimate goal is to move the clients forward even further into self sufficiency. Having strong partnerships with many landlords in the Atlanta area, AOPI has the ability to move many clients into their own independent apartment or housing. Because we are committed to our client’s success, AOPI follows up with them even after they have moved on, and remains as relational support in their new environment.
